How Do I Sharpen up old B&W and enlarge

My father is being inducted into our high school's sports Hall of Fame. They need a 5x7 for the plaque and my father found the attached wallet. I need to enlarge to a 5x7 and sharpen/clean it up. I have CS2, but I am definitely a beginner on it. I can follow directions. I also only have a couple of days to accomplish this. Help!!!!!

It's all done in adjustment layers, so you can DL the psd and tweak it to your own liking. In the future, you might want to link us to the full sized scan as well as the small pic as it may turn out better restoration results.
