Model Release/Customer Agreement Forms

So I am currently deployed. I am trying to take care of as much stuff as possible in terms of launching a small part time photography business. I have already lined up a good amount of clients when I return from where I am, and I am in need of a model release/customer agreement form. I am curious what you guys use or what kind of things you say in yours. Any bit of info would be greatly appreciated!

I sent Ryan a Model release, but I think he could still use a standard customer agreement for casual portraiture and the like.
