Permission to reprint form

Can someone who has already lawyered up please furnish me with the basic outline for allowing someone to print your photographs for non-commerical purposes? I just finished my first wedding on my own, and I gave the CD to the bride. The local camera store wants a 'copyright release' (on company letterhead 'like I have that') I know that I do not have to give her a 'copyright release' to allow her to print photos for her personal use. I just don't want to try to write something myself and have them not except it and have her come back to me like I am an idiot. Thanks, I know someone here can help me out with this.
