I did a blurb book a while back and was happy with it.
Schmooo(zer) has some experience with them as well.

We've only done small books, but have had good luck with Shutterfly.

I'd go with Blurb. Now they have the premium paper option, which uses higher quality paper, but it's limited to books with 160 pages or less.
