Soliciting feedback from customers?

Can anyone share some of the ways they've solicited feedback from their clients? What kind of questions helped you receive responses that were most useful to your business?

    One creative way would be to have a "feedback" or "satisfaction" form that you could give to your clients upon completion of the shoot with a coupon for 1 free 4x6 or 5x7 print with their (or future) order.
    Most people are eager to fullfill this if it is easy and they get something tangible in return.

    I'm planning to use the end of the year as an 'excuse' to send out a short questionnaire to all of my 2008 clients.

    A few reasons for this:

    1) A handy reminder that I'm around and to bear me in mind for future work
    2) I'm really bad at following up post-event; chasing invoices yes, but I otherwise not much more than "What did you think of the photos?" "They were good/great!". I genuinely want to know what people are thinking of the quality, price, service - I think I'm reasonably priced, surpassing client's expectations for quality and providing decent, but not exceptional service (not quite to Smugmug's standards :D!) but I can't back this up (except that people come back to me). Maybe they are all secretly cursing me.
    3) Hopefully a good way to get some quotes/testimonials for the website.

    I can throw in a free shoot for a randomly-chosen person who completed the questionnaire as an incentive.
    As for questions - more elegant versions of
    "what did you think of my price"
    "what did you think of my photo quality"
    "what did you think of my service"
    "would you recommend me to a friend"

    Hopefully that will give me an idea of where to concentrate improvements...
