I guess I would like to know what the intended significance or application would be for the image.
Whether it be portrait, commission, having to do with a larger project or theme, or just experimentation...
As it has a certain vibe going on.

The actual technique itself is very well done.
Good use of textures and post-processing.
The model is cut out very well.
    Nothing over or under-exposed.

My only suggestion would be perhaps cropping the bottom 40 pixels or so out of it, so that less would be visible of the hands. I think it would bring more attention to the eyes and eliminate some distraction. Of course, that might just be personal preference.

    A quality image :)

First thank you all for your comments, it's greatly appreciated. There's really no intended significance or purpose to the image. Just getting in some practice, been working way too much and have had photography in the backburner for a while, I need to find a balance between work and passion I guess.

The shoot literally lasted 5 minutes including setup and took a whopping 2 shots :) All my model (wife) had time for, it was bedtime :)

Like mentioned before used 1 AB800 with shoot thru camera left about a foot from the model at 1/160 @ F/8.

Used 2 textures to create the effect, one in overlay mode one in multiply mode. Masked out areas as needed.

    Always an important endeavor :)

Well, a great job for a pre-bedtime 5 minutes, certainly, based on all the feedback you're getting :)

I'd say an imitation of the head shadow on the scriptures would help, so would some blur.

Wow didn't think a 5 minute image would get this good a response lol. Thank you for the critiques and comments.

Question for the NAPP members out there, is it worth the membership fee? I've always thought about joining but I want to know if I'm just gonna be rehashing things I've learned already. What difference does NAPP techniques have as opposed to readily available info on the web?

lol and I just realized that I titled this image after "Extreme", I highly apologize :)

It's a good resource to have. I don't use it often, but I do use it sometimes. If your annual budget is less than $1K - fuggedaboutit deal.gif , otherwise it's only a single Benjamin (which rolls nicely to your business expenses, thus making it about only three Jacksons:-)
