Just what I was going to say! It's not even necessarily a vanity issue; it's a professional issue if you are going to show this (on your web site, or whatever). You would do this for your clients -- do it for you, too...

'Tell the model to smile' Oh boy! If someone isn't there making me smile when I do these.....I always look like I'm in pain. rofl
This has the makings of a seriously great photo. Hot lighting? Maybe, but you "chose wisely" in your treatment. It really works. I'm not even worried about the lack of a smile. Sometimes, it's just not the right thing to do. Methings, this may be one of those times.
Some things I would address:
Crop from the right - extend to the left.
Loose the knee (or whatever it is) in the lower left.
crop a bit from the bottom to loose the button on your back...and the bit of sleeve at camera left....and then crop some from the right to decenter it and.....
You look great! I too love the treatment and I'm not a fan of sepia normally. I may tilt it a bit so it doesn't look like you're craning your neck back quite so far. I also agree w/ getting rid of the buttons, knee, etc. that Scott mentioned.
