Password prompt submit button missing on IE

Go here to a password prompt in IE7:

I (and others) see no submit button in the password dialog. When I look into the CSS, I see that the submit button has a zero height. If I force it to have a height with some custom CSS, the submit button appears.
Note, this URL is a generic smugmug.com URL so I believe the problem appears even without this user's customization.
The original report of the problem from a customer was in this thread.
FWIW, this does not seem to be happening in the password protected prompts on my site. I don't know why it would only happen to some sites. Also seems to work fine in Firefox 3 and Safari.
Since I use a lot of password protected galleries and have just recently published more than 50 of them for a large school produection, this has me personally quite nervous.

The user's site had a theme applied, I fixed his code there.

Andy, as you will notice I said in my posting that the issue wasn't happening on my site.
