Anyone have any problems with Black Rapid Straps

I was reading this thread. i use a Black Rapid strap and love it. But the discussion in that thread got me wondering - has anyone had a problem or dropped a camera using one?

There were a few inconsistencies with the early generations of Black Rapid's straps. The most common point of failure was with the screw that attached to the camera -- it would back out and the camera would drop. There were also problems with the fastener coming undone and dropping the camera. I wouldn't trust one of those new ConnectR-2s either from the looks of ziggy's post.

I don't own one of the straps as I made my own for far less money than they charge. I did, however, buy one of their FastenR-2 thingies. The rubber o-ring really grips on the camera body and the bolt making it a little difficult to get off on purpose. I haven't had any trouble with it backing out.
