The hand orientation feels much more "A Clockwork Orange" than medical professional. Not sure what what to do. Perhaps just a thumb raising the upper lid? I get the OOF aspect and why you want it that way.

That's my hand and right eye.. (I injured it when i was 17). and besides it's already in medical books from the wierd scist that formed after the 3rd surgery. But i was going for creepy / dark .. think it helped in BW
