Order form?

I am trying to design a new more proffesional looking order form that shows all of the options that I offer. It's prooving to be a challenge. I have a chart with the print size at the top and the paper and finish options down the side and the prices in the chart. Maybe I could add empty rows below the prices for the customer to put the file numbers they would like to order. Any ideas? Examples would be great.

    As far as customers and forms, keep it simple. Eliminate options if necesary.
