It will depend on the ISO setting, exposure, image content and other factors such as whether the noise is colour (easier to fix) or luminosity (harder to fix)...however third party noise reduction tools often beat Photoshop or Lightroom and there are many competing products on the market to try. I personally like "Noiseware", however there are many others (Define, NoiseNinja, GrainSurgery etc).
Noise may also be more apparent in some channels more than others, which can sometimes help for channel blending to reduce grain/noise.
