Someone wants to use my photo for an album cover. Now what?

Someone just contacted me asking if one of my photos is available for use as an album cover. I would love to have my photo used on this album, but I have no idea how to handle this transaction. Could someone please give me advice on pricing, photo credit (I definitely want my name mentioned with the cover art), and any paperwork I should have signed for this deal.

Does the photo include any people? If so you'd need a model release. Can't help you on pricing, other than don't give it away in exchange for "credit". Credit doesn't pay the bills, and seriously, what are the odds the photo credit would really bring you any future business?
