Deleted Photo Now a Photo Deleted Thumbnail

I have deleted a photo from one of my galleries, (of which I have had no issues prior doing the same task) but I am now seeing a "Photo-Deleted" thumbnail where the photo once was. How do I rid of this "Photo-Deleted" thumbnail and prevent it from appearing again with more maintenance (deletions) ahead?
