Model release for re-sale.

I use to have a model release were if you paid the model to do work for you you could re-sale the photos for a calendar or what ever you choose to use or sell the photos for. I never used it and have lost it. The release I have right now is for TFP and is or will only allow me to use it for advertising. Does any one have one? I had got them off the web site for photographers but that one I need is no longer there... Just the TFP.
