Might be a Hogna carolinensis -- let me know if my Hack guess is wrong.
Tamron 60mm macro hand held, some with only built-in camera flash some with an off-camera SB-600 assisting.

Gotten a couple of questions about this. Here is a 100% crop of one of the shots. Was shot with the Tamron 60mm with no "add on" attachments (tubes, TC, or close up filters). You can see how shallow the DOF is at 1:1 distance.
