Van Gogh Ear Portrait

So I have an artist friend who just had a mole removed from her ear and now has a giant bandage over the whole ear. She thought it would be fun to do some portraits a la Van Gogh with her bandaged ear. He sent his to his lover, she sent hers to pathology. Anyway, I think that done well, this could be pretty interesting. Done not well, could be kind of... not good :)

So, at the moment I'm thinking of going for a dark cavernous background, warm light on her face, and a serious expression.

Does anyone have other ideas/thoughts/suggestions? I don't think it needs to be a replica of the painting, but it should convey a similar mood, maybe even a darker/more severe mood.
