Photos missing from pages, showing red X >>> custom sizes

My smug site is quite unhappy right now, and I have to admit I am less than happy myself. This problem has happened before; it's been quite a while, but it should never happen.
I saw the problem this morning before I left for work but didn't have time to report it. I saw it during the day when I went to my site. And now I see it quite often. Refresh the page enough times and sometimes you'll see a page without red x's - but more often than not the red x's are there.
I use <img src= statements to display photos in many places on my site. This includes my homepage, my guestbook pages, my kaleidoscope page, and the galleries that I use to replace the standard smugmug category and gallery pages.
Most use random images (using the code shown in this post. All use custom sizes, as shown on this page - http://www.smugmug.com/help/custom-photo-sizes. Given that I've seen this on photos displalyed using a specific key as well as a random photo, I suspect the problem is in building the custom sizes.
I have not seen the problem on standard gallery pages.
It happens on HTML-only pages - and my site is full of HTML-only pages.
