Working with a "fixer" (no photos)

I'm interested in hearing experiences from others that have worked with "Fixers" when setting up or actually out on a shoot. I'm usually a loner when taking pictures, but there are so many situations that having someone around that could help with literal and figurative interpretations and setting things up that I'm thinking about recruiting a "caddie."
I know this is common practice in PJ work, almost a necessity. What are the primary things you look for. What's the adjustment like having someone with you when shooting. What are some ways that having a fixer has helped you get something that you wouldn't have otherwise?

You mean a second camera
someone to carry gear
or someone to occupy subject attention so you can shot without distractions of that sort.
I do use my wife occasionally for the latter but mainly wing it alone on the mean streets of L.A. :wow
However if I ventured to unknown lands/territories I would us a fixer.
Don't know if this helps of even begins to answer you original question, just what came to mind
