Your catchlights seem to indicate a rather large softbox or two really close to one another....
I did use 1 large softbox and it was very close due to space so the catch lights are huge!

No problem, check this out and you will see exactly how we shot these and all the equipment we use;) for a quick answer, I think it was the Canon 70-200 f/2.8 L IS. That is usually attached to the 5D Mark II.
