Pricing for Commercial Use (website, print and display)

I am an amateur photographer. It's my stress relief outside of work. I have a branding/advertising agency that saw one of my photos and has a client (Real Estate title company) that wants to use it for:

1. Display on company website representing the area
2. Large print to hang in their office
3. Advertisement - 1/2 page add for company

I have no idea what to charge. I've tried searching around but haven't found much guidance. Also, is it customary to ask for photo credit in any of the three instances. The agent said in commercial photography one doesn't list photo credits.

Also, would I provide the agency a high-resolution file for the print and a lower resolution for the ad and website?

Any help is appreciated!

    I appreciate your reply. How did you get to the amount? Does it matter how long they want to use the photo or how many times they want to run the ad?

    Make sure you limit the use for the medium and duration they require
    everything else is a free ticket for them if you don't price accordingly.

    How about you calculate how much time you spent creating the images
    and add an ammount for the value that you place on yourself or your
    photographic abilities?

    I would look what other photographers charge and what their packages
    are so you get an idea of the price and quality range.
