DVD covers

Trying to make my own DVD inserts but I am having trouble with them. Does any one know the proper dimensions or of a downloadable template for Photoshop?

    They have a um .ah. bunch of these at most computer stores like Fry's. They sell kits for CD's and cases with templates, and perforated sheets.

    Very cheap.
