Printing Problems with Bay Photo

I recently ordered a batch of prints from Bay Photo and there were some problems. Do I consult SM or Bay Photo to sort out the issues?
I ordered every photo on my website as either a 4x6 or 4x5 glossy. Problem is they didn't send the 4x5's even though my SmugMug order confirmation has them listed. Why?
Upon reviewing the photos that were sent, I noticed many of the prints had highlights that were blown out. I compared these to prints I had made a couple months ago and those do not have blown out highlights. No changes were made to the files between print runs and the gallery has no color correction set. Why the inconsistent results? Was the printer out of calibration?
I have branding enabled. The sticker that was attached to the package looked horrible! The sticker was supposed to be black with a white logo and text. What I got was a washed out gray with white lines streaking though it. There was obviously a problem with the print head. Ironically, the text on the sticker says: "All Prints 100% Guaranteed". I put this statement on the sticker to emphasize to the buyer that they can rest assured the prints will be of the highest standard. But what does is say when the custom branding is of a very low quality? I'm glad this didn't go out to a client!
I'm attaching scans of the sticker and a comparison photo to show the highlight problem. Keep in mind these are scans and the problem is much more noticeable when viewing the glossy prints.
I've been pretty happy with Bay Photo in the past so I'm hoping this is a one time occurrence. But at the same time I'd like some answers so the problems are addressed and don't happen again.
