One little, Two little... files

In the link below the first 2 photos are the same. I'm not sure what I got screwed up here but if I delete one they both delete (done that). Then I uploaded the file again hoping that I would only have 1 file.. but again it was there 2 times? It has the same filename and.... well I'm lost. Any ideas?
