TXS Josh, The set-up is rather simple.
First I make-up my mind on what I want to achieve in a picture. Once decided that I wanted a dramatic picture with a dark threatening sky the work can start. Look for thunderstorms or other impressive sky's. Take the shots of the clouds and store them in a library for future use.
A then try to line-up the models color of skin and clothing/attributes with the pallet of the sky. Apply in the studio dramatic light (lots of shadows) and line them up with the light in the sky picture, so that it looks a bit real from a light perspective. I used a black background for the picture with the girl. Putting 1 and 2 together is CC job.

If I look to the right, there's a halo in the clouds around the right side of her head. Also, seems unnatural for her to pop out of the clouds like that.
