Custom Forms That Are Working

Using Adobe Forms Central, you can have pretty much all the custom forms you want. You can even setup payment fields on the forms. You can't embed the javascript but you can add a URL link to an image.
Yes, the service cost $143 bucks a year so it ain't free but it works. Don't yell at me because I was willing to spend the money. You can see it on my website under Purchasing. Its still a work in progress. I did not include the payment fields and links on the form but that was just a personal choice. I believe they would work if you did.
It appears to work on iPad and Samsung phone, as well. I haven't seen a better option yet in the forum, but if there is one, let me know.
