This appears to be a genuinely light-skinned subject along with the use of layers and a high-key treatment of the skin as well, leaving the clothing as normal tones. The two layers, high-key and normal, are then blended selectively by tone in Photoshop.

Could also use a very careful selection mask for the skin tones to provide the separation.
