Help with creating a contact page

Is there a easy way to add a contact page? If so can you please help me. I think I found my answers. Thanks

Go to Organize
highlight your name in left tree at the top
above click create > page
go to new page
at top customize > customize site

in right flyout highlight "just this page"

click "HTML & CSS" at bottom under content tab to expand it
slide a HTML block out to page.
click wrench on it
add in all you html/info in the basic tab.

This is what Ive been trying but when I click the Done button nothing happens. It won't close out the window. Why is this?

To use the SmugMug contact form, you have to have a customer contact email entered in the user profile AND verified that email address by clicking on the link that is sent to the email after entered the contact email.
To access your user profile, click on the profile thumbnail in the upper right next to the search bar on your homepage.

If you still have an issue, please include a link to your site so we could take a closer look.

There is an option to add your own custom form to the page itself, however it's a bit fiddly and you have to use wufoo.com. Then there is a Wufoo content block where you can add your form. Unfortunately, it doesn't match the Smugmug formatting brilliantly with buttons, etc. using the default web formatting.

It's directly on the page. I use it on my website.
