Guest book with ability to insert photo near coment

Dear Colleagues, please give ideas how to make a guest book, with ability to insert photo near each coment. For example: a guest left a message, then the administrator adds to this picture and post it. I enclose a picture of how I see it.

I don't know of any way for you to alter the photo in the comments.

What about if you add a guestbook for initial commenting purposes only plus create a page on which you will place the selected photos and comments? When someone comments, copy the text and place it in a text box on the new page, then place a photo next to the text.

Two HTML content blocks (one is link to Wufoo form and the other one is a comment text with photo) It work so: Wufoo form will email me customer's comment, in next step I will modify HTML code adding new row with text and photo.
