Gallery Thumbnail go generic after editing folder contents

On several galleries which originally displayed an automatically selected thumbnail from the folder contents, now displays the three-overlapping-boxes icon. In each instance I had Copied and image and them Moved the copy to a different gallery. The copied / moved image was not the one being displayed. I have 4 folders stuck in this state, and can reproduce the issue after copying just a single image. The thumbnails are missing in both the public view as well as when I login and use any of the views which show gallery thumbnails.

(A) is there a way to fix this? I've tried noodling around by changing gallery styles and so on.
(B) can you fix this on your end? It seems very easy to reproduce.
(C) If I'm doing something stupid, can you tell me what it is? I'm trainable.

Did you try setting a "featured image" in gallery settings?
