What am I doing Wrong?

My photos lately have been looking soft. Can anyone help me figure out what I'm doing wrong?
The image was shot at
1/160 sec at f/5.6 ISO 200
70mm

    The image was really degraded when I had to reduce the size to upload here. But there is no sharp focus on anything. My focus point was on her face. I shot in RAW.

    Looks more crooked than anything. Sharpness looks ok at the posted size, but a larger sample, preferably full-size original is really needed to evaluate further.

    It fell off a prop a while back but I have only had the problems just recently.

    Have you done any tests with your camera and lens(es). Set up a stationary item, put your camera on a tripod and take some shots both with auto focus and manual focus. Do this with multiple lenses so you can determine if the issue is the lens or camera.

    Just do a simple focus test:

    Place the camera on a tripot first ,autofocus and RAW:

    1.Select a single focus point.
    2. Use f 2,8 or lowest number for your lens
    3. Keep shutter at 1/60sec
    4. Keep ISO as low as possible
    5. Place a 1 food ruler under a 45 degree angle in line with the camera's sight on field at a distance of 4-6 feet (pending on your lens)
    6. Focus on the 1/2 food mark on the ruler.
    7. Make the shot
    8. Check in LR where the sharpness lays on the picture of the ruler...it should be at the 1/2 food mark

    If it is not try again, check again and if the sharpness is closer or further away from the 1/2 food focus point then you might be able to adjust the fine focus for that lens within your camera menu (pending what type and brand you have). If not possible then either your lens is bad (try another) or your distance metering system is faulty in the camera....
