Viewer input their name and e-mail address prior to entering the sub-folders

I have a folder that is password protected, it contains other folders and galleries. I would like viewers/visitors to input their name and e-mail address prior to entering the sub-folders. So I can have a list and know who is visiting.

    You might be able to add a Wufoo form to the password page. But it would show on all password pages.
    And you can't be sure that they would use it.
