NEW TUTORIAL: How to add a PHONE NUMBER to the standard SmugMug contact form

Hello Friends!

End of the week so it is time for a new tutorial.
I have seen quite a few people who asked if it is possible to add a phone number to the standard SmugMug contact form - now there is :}
Although the default SmugMug contact form has very basic customization option - I have written something short and easy to use, that will allow you to add a phone number to your contact form:

I love your tutorials, however I am trying to use both this and the white contact form and only the white is working. Is there a trick to get them to work together?
