Are you referring to when you view a photo in lightbox view? You can turn off titles, captions, and keywords from showing in the gallery's lightbox view. First go to a gallery. Then customize > content and design > upper right select either All Galleries or Make This Gallery Custom > content tab on the right locate and hover over 'lightbox' > select the wrench to the right of lightbox > in the lightbox settings, turn off titles, captions and keywords.
This page may be helpful as well :)

I just did a quick test in one of my galleries. I turned the Show Filenames property on the appearance tab of gallery customization to ON. When I looked at the photos in lightbox I see the filename. I then turned Show Filenames to OFF and checked again. The filename no longer shows.

Can you run a quick test on one of your galleries? If Show Filenames is set to OFF, turn it ON, save the settings, then turn if OFF again. Does that make any difference?

Have you verified that the file names aren't actually saved with the photos on your smugmug site? When I look at the gallery http://www.patrickdesouza.com/Galleries/Bird-of-Paradise/ I see that some of the photos have the file name as title and some don't. That implies to me that it isn't a setting issue but is that the filename is actually the title on some of your photos (unless the photos that show the title are actually collected into that gallery?

If the filename is placed in the caption field by setting show filenames to ON, when you edit the caption it will show as blank. Here's an example from my site:

If the file names are stored in the title or caption on your smug site then Kelly's suggestion will hide them in lightbox.
