Best Solution for Surveys on SmugMug

What have other folks done to implement surveys on their SmugMug sites? I know javascript isn't supported so any survey tools that generate a js block will not work. Ideas/suggestions?

It appears that many of the survey tools use JavaScript.

The only forms that are currently supported are Wufoo forms. When I did a search for "wufoo form for survey" I was taken to a Wufoo page of templates showing surveys - https://www.wufoo.com/gallery/templates/surveys/.

If the Wufoo surveys don't work for you have you considered linking to a survey offsite?

Thanks @denisegoldberg, I did look at that Wufoo page but unfortunately I have used up my 3 free wufoo forms. Upgrading to a paid wufoo account is a little steep for me. I'll have to look to link to an external site.
