MailChimp Signup Form - Any one had any success???

I have been researching using a Custom MailChimp Signup form on my SmugMug Site without acceptable success. I was expecting to see my MailChimp CUSTOM form when Clicking on REGISTER NOW but only see a miniature basic form.???
Has anyone had success in this procedure? if so I would really appreciate seeing your sight. Mine is "sunnypicsoz.com"
