Webform question

I am wanting to add a questionnaire that I can customize that once a client fills it out, it is emailed to my email to my smugmug website, but am unsure where to get one and how to apply it to my website. I am hoping someone here can provide me with some advice.

The only form "compatible" with SM that I know of is a MailChimp sign-up form.
