Looking for a usage contract form

I have a professional performer who would like to use one of my pics on a promotional T-shirt. We have yet to agree on the terms of the deal. But I would like to find a contract form that I can use to formalize the agreemnet once made. I'm new to this type of sale and have no money to employ all lawyer. So I would like to find a stock form that I can "fill in the blanks."

i guess i did not use the correct terms. because i tried that...
