Reverse Images?

What's the cause of reverse images and how can it be prevented? By reverse image I mean it's just like flipping a negative upside down. Has anyone else had this problem?

Nope, the images were imported from a memory card to my computer via Photo Mechanic without any kind of processing. They were then uploaded directly to SmugMug. I have subsequently viewed the images on the hard drive and they appear normal. The reversal appears to have happened during the upload process. I'm using Mac OS X.
