Went back to the same spot with a cleaner sensor and added a 2X TC.

Looks like some of the shots are taking a hit in diffraction -probably due to the 2xTC. Do you have a 1.4 or 1.5x TC and tubes that would give you the same magnification? You'll get better image quality if you take out the 2xTC -I tried one made by Tamron and went back to my Canon 1.4x and tubes...

Through the viewfinder, I couldn't really tell what was in his mouth. I got off 2 shots before he had his back to me.

I thought they were pretty crisp for being hand held. 200mm hand held laying in mud could be the problem, but you may be right. The 2X is all I have. It is the more expensive glass from Nikon though. Can you explain what artifacts you're refering to? It might help me in choosing accessories in the future.
I do have a set of extension tubes (12mm, 20mm, 36mm) but haven't used them all that much. I used the TC in hopes of getting similar shots from a greater distance, but it's always hard to avoid getting as close as possible.

It helps to have a lot of targets. The little patch of flowers on my property has had at least 10 both times I went out to shoot. It makes it easy to switch subjects if they turn out of the light or aren't in a good position.
