Photo Delivery Receipt

Can anyone explain these to me? A particular organization that I would like to submit photos to has requested a Photo Delivery Receipt. I have been unable to find adequate information and resources as to what these are, what they look like, and how I need to fill these out. Any advice is greatly appreciated. Thanks everyone! beyond that I would simply ask them for further information as to what exactly they want as you don't want to assume anything and make a mistake.
