Keyword and Caption Weirdness

Hello-

Not every time, but often enough now, I will keyword and caption a photo and hit save and everything looks fine. When I go back to the photo later, it is all garbled like this:

I don't get it. I'll go back and correct it and save again and sometimes the corrections stick, sometimes they don't.

Any ideas?
