Santa shoot - question about paperwork

Hi guys,
I'm doing a Santa shoot this weekend (kids posing with Santa at a local museum). I have a quick question about the paperwork.
My plan is to offer a $5 5x7 print & upload the image to the web for possible reorders. I do not have direct printing, so the print will not be available until the following week.
Here's what I was thinking of saying on the flyer that I hand-out to each customer (this is a rough draft...just thinking as I type):
"Please return to the gallery next Saturday between 9-5 to pick up your 5x7 print...or print can be sent via mail to you within 7 days for an extra $2.
(does charging that extra $2 seem tacky?)
Additional prints can be ordered online at www... "
Also, I assume this will be a yearly event, so I'd like to use this year's images for next year's advertising. What type of blurb should I put on the customer form that will work as a model release? I'd rather keep it short, I don't want the people to have to read a long contract.

Thanks for the info...I thought about that option, but then I'd have to charge another $5 to cover the SmugMug shipping costs.
To give a little more details about this shoot, we're actually charging $10 for a 5x7 (the museum gets a $5 cut, I get a $5 cut)...so I think adding another $5 to the cost would be prohibitive.
Did you mention anything about model release terms on the address form?

I didn't do anything about model releases, as I wasn't expecting to use those portraits as promotion. Looking back, that's something I should have done.
