wedding album price..? what do you charge?

I have a client that wants a formal wedding album? I have a few questions:
1) how many images do you usually include in teh album? (so, how many pages?
2) what size images do you insert into the album?
3) what do you charge? or...what would you charge if this was an add-on option?
4) Where do you get your albums? I want to keep the printing through smug?
Any/all info will be greatly appreciated.

If you are talking about a matted album, where you take an existing print and insert it into a page that has a mat, I can't speak to that. Those have declined considerably in popularity and are (IMO) somewhat complicated to price as the assembly requres lots of different elements. Try Albums, Inc.
I personally design my own layouts for use in Asuka coffee table books (press printed page), and WillowBook (Sequoia flush mount albums, where the layout is printed to a photograph and mounted on a thin cardboard page).
I could tell you what I charge but it won't mean much to you until you choose a product and factor in work time, which varies considerably depending on the style.

1) depends on whether it's a traditional or flush-mount digital album
2) this is in traditional albums only. usually i go for the 16 or 20 spline albums = 32 or 40 prints
3) typically an add-on
