My Erase to History Tool is Misbehaving ...

I have two things going on with my eraser tool (Erase to History):
1. Intermittently, it seems, the Erase to History check box is grayed out and unavailable. I've checked to make sure I'm on the right layer, checked to make sure I didn't have anything locked, and checked all the settings I know of but can't figure out why it's unavailable. One time I randomly changed brushes and it became available, but that was just once and that solution hasn't worked since.
2. It used to be that if I brushed over a part of an image that has not been erased, there would be no effect on that part of the image. Now, when I brush over an unerased portion of the image, the Erase to History brush leaves a dark coating of sorts on the image. What's up with that?
As I said, I've checked all the settings I'm aware of but to no avail. Maybe there is some obscure setting somewhere that I'm not aware of? Or, maybe I'm overlooking the obvious? I'm feeling kind of stupid, but frankly, I'm stumped.
Oh yeah, it's Photoshop CS.
