Commercial use question(s)

I've been doing event photography for a couple of years and have sold the commercial rights to a few photos for up to $300 each. I've recently been asked for my price on using multiple photos for a DVD + DVD cover for a recent local event and I don't expect this to be a huge seller, so it probably will be a minimum production number. Being part of this sport and knowing these people well I don't want to be taken advantage of, but neither do I think they'll want to pay my "normal" price for this use. Not quite wure how to price and avoid being taken advantage of.
Does anyone know what kind of resolution I should provide for this use? Do I sell them a limited use license and if so is there any forms out there that I can use for this? I'm not experienced in this area of photo use and would appreciate any advise!
