I removed the one from the custom header; however the problem remains the same.
It's working reliably here now in every browser I tried (Chrome, Firefox, Safari) except IE (where the feature isn't supported by Smugmug). You might try clearing your browser cache and then restarting the browser and starting with a clean state now that you've made a change.
Is there a reason why the Easy Customizer favicon would work on some pages but when you drill down to the specific gallery level, it reverts back to the SM favicon (e.g. clicking any of the three left links in my header)? http://www.nawset.com

Smugmug prevents implementing a reliable way to set the favicon because their own favicon is always in the web page first. Your custom favicon works when it works and does not always work. That's just life until Smugmug decides to actually make this feature work reliably. Nothing you can do about it.
